Directions:
1. Registrants and volunteers! Please download and fill out the appropriate forms from the links on the right.
Church group leaders! Please give each attendee an application to fill out AND also fill out the "Church Registration" form.
2. The attendee application includes an application, personal injury waiver, and media release consent form; please follow the directions on the forms and fill out ALL THREE (3)!
3. Then mail all three forms* and correct amount of checks/money orders to:
